IF YOU USE THE LAST MILEAGE LOG GET IN THE FILE CABINET AND GET A NEW ONE. DO NOT TURN IT OVER AND WRITE ON THE BACK. THESE POLICIES ARE NOT THREATS THEY ARE CONDITIONS OF EMPLOYMENT. OUR EQUIPMENT IS GETTING ABUSED AND IT WILL NOT BE TOLERATED. January 2007: Employee Uniforms / Dress Code: Effective January 1, 2007, Longwall
West Inc. has changed our uniforms.All employees should have have
received their new shirts. All Employees also need new pants. You can
choose from the following options:
Bridger runs:A.) Charge $100.00 worth of new jeans on the company credit card. This needs to be done by Friday 1/1/07.
You will be given money to purchase new jeans at the beginning of each
new year. Also, the jeans will be put on a quarterly depreciation
schedule. This means if you quit before the end of the year, you will
owe LWI back for some of the money we gave you for the jeans. This will
be done because the jeans are for you to keep; they will not be turned
back in to LWI in the event that you do not finish out the year with
us. They will be depreciated at the rate of $25.00 per quarter. ( So,
for example: You only work here for 6 months, or two quarters, and then
quit. You will be payroll deducted $50.00 out of your last check to
cover the remaining two quarters worth of jeans that you did not work).B.) Receive three (3) pairs of Carhart Double-Front jeans from Longwall West Inc. ( These are jeans like the Fairmont Supply employees wear.) You need to have your sizes into Brian by Monday 1/15/07. All employees need to look presentable when they come to work. We understand that you will get dirty from loading / unloading. However, you need to have clean uniforms on EVERYDAY. The way you look portrays the overlook and professionalism of the company. We want to send a good message to our customers, and this is mainly done by YOU, the drivers. Effective immediately, all Bridger runs
need to have a copy of the fuel receipt(s) attached to the bill of
lading and /or Omega ticket. Either get a duplicate receipt upon
fueling, or make a copy in the office. The original receipt needs to
remain with the fuel log in the vehicle log books.
On all Omega tickets attach a copy of the fuel receipts to those, too. Calling In: All drivers need to be sure you are
calling in with your times! You also need to be calling in your time of
delivery at the mine in case there is a backhaul. Use the warehouse
phones and our tool-free number if you don't have cell service.
Vacation request forms:Effective January 1, 2007, all vacation
request forms need to be turned in at least three weeks in advance.
Forms not turned in before three weeks of the requested days may be
denied!
Ohio D.O.T.:If you have an emergency (or are sick, etc.) and need a day off without notice, a personal day must be used. Paperwork must also be done for these days. Any time we have to drive through Ohio
with a special hauling permit, the driver must have a copy of the OS-1A
Provisions/Limitations form. This is in the office. Make sure to get a
copy before you enter the state of Ohio.
Meal Policy: The current meal policy
states that LWI will pay for a meal if you have to stay the night or
are out on a HOT-SHOT run longer than six hours with less than 30
minutes notice. If you are scheduled for a run a day in advance, meals
are not covered (ex. can runs, etc.)
Company Cell Phones:
Effective December 21, 2006, everyone
will have a total of 150 peak minutes per month (this runs from the
21st to the 21st) to use their company cell phones. This equals about 7
minutes a day, 5days per week. Everyone has unlimited walkie-talkie
minutes and mobile to mobile minutes to other company cell phones, and
unlimited nights and weekend minutes, (this starts Friday after 7:00 pm
and runs to 6:00am Monday morning). Text messaging is not included in our plan.
Any charges in excess of 150 minutes per month will be payroll deducted at 40 cents per minute. Any questions can be directed to Brian. Plug-in all diesel trucks at the end of every run. Car wash tokens are available at the office, and are only for use on company vehicles. Acquire prior approval for getting trucks washed at commercial truck wash sites, the company will not be responsible for paying. BHP pre-trip inspections: need to be done before leaving the yard
every time you go to BHP! By doing this, items that do not pass
inspection can be fixed and it will prevent having problems at the mine
site. BHP will not allow entry if there are safety concerns. This can
get very costly.
Joy paperwork: All Joy paperwork needs to be returned
to Joy upon completion of their runs. If they give you paperwork for a
run they send you on, you need to take it back to their warehouse when
you get back.
Market Express Fuel Punch cards:Do not accept frequent-fueler cards
punched when fueling company vehicles at Market Express locations. LWI
contractually receives tokens on a monthly basis. If you have been
accepting punches, STOP IMMEDIATELY!
